If you live in our practice area and would like to register with us, please complete one of our questionnaires that are available from reception, and bring along your Medical Card. If you do not have a medical card, then our reception staff will provide you with a registration form to complete. You will also need to provide photographic ID (passport/driving licence) and a recent utility bill in your name identifying your address. You will then be registered with our practice and can see any of our GPs.
The practice can refuse an application to register, only if they have reasonable grounds for doing so. The practice will inform the applicant within 14 days with the reasons for refusal. Our Practice welcomes all patients and does not discriminate on the grounds of age, appearance, disability, gender, medical condition, race, religion, sexual orientation or social class.
If you live within our boundary but are registered with an Out of Area Practice that is participating in the Patient Choice Scheme, you be able to access our primary care medical services. Please advise reception if this is the case.
