**CAMBOIS SURGERY UPDATE**

Our Branch Surgery located in Cambois is currently up for sale.  An auction will take place on Wednesday 1st May 2019.

We will continue to provide medical services to the residents of Cambois.  We are uncertain if our location will remain in Cambois or if services will be provided from Bedlington.

We would like to reassure our patients that we will keep them informed during this process and hope for a quick resolution

Please contact the surgery on 01670829889 or 01670823917 if you have any concerns

New Patient Registration

If you live in our practice area and would like to register with us, please ask at reception for one our registration packs. You will need to provide photographic ID (passport/driving licence) and another form of ID in your name identifying your address (e.g. a bank statement or a utility bill). You may then be registered with our practice and can see any of our GPs.

A practice can refuse an application to register, only if they have reasonable grounds for doing so. The practice will inform the applicant within 14 days with the reasons for refusal.

Our Practice welcomes all patients and does not discriminate on the grounds of age, appearance, disability, gender, medical condition, race, religion, sexual orientation or social class.  You may request a new patient health check once registered with us. If you wish to do so please advise a receptionist.

If you have a long term condition (LTC) you will be invited annually to review your health and medication.

Please note it is practice policy not to allow children to be regsitered at the practice unless they also have a parent or carer registered with us at the same address.

Change of Details

Please inform us immediately if any of your details change, i.e. address/name/telephone/mobile number.  It is essential that our records are kept up to date in order to provide you with the best service.  You must change your details at reception and bring some proof of your new address.
